How big are the small, medium and large squares?

What is the real-life square mileage of the three different square sizes?

Well, each small city tile is exactly 1 km x 1 km, medium tiles are 2 km x 2 km, and large tiles are 4 km x km.  Some simple conversions (1 kilometer = approx. 0.6214 miles), and you have:

Small Tile = 0.6214 x 0.6214 mi., or 0.3861 sq. mi.

Medium Tile = 1.2428 x 1.2428 mi., or 1.5446 sq. mi.

Large Tile = 2.4855 x 2.4855 mi., or 6.1777 sq. mi.

Yes those are correct however, if you take a look at close look at all the props, buildings, people and cars, you will notice that the scale is quite off. When you crete buildings which are to real world scale, it looks kind of small when compared to maxis standard game material.

I thought it wasn't quite 1km exactly - each tile is 16m square, so the side of a small tile is 1024m (16x64), medium is 2048m (16x128), large is 4096m (16x256).

From what I've seen, things are about 4 times taller than they should be using the horizontal scale.

The following information has been gleaned from posts by moganite of the New Horizons Productions Team and from dedgren, author of the Three Rivers Region City Journal.  I have rewritten it for ease of understanding / reading.

A single grid square in a city is 16 meters square.

A small city tile is 1 x 1 pixels.

A medium city tile is 2 x 2 pixels.

A large city tile is 4 x 4 pixels.

Consequently:

A single grid square in a city is 16 meters square.

A small city tile is (1 x 16m) x 64 = 1024 meters or 1 kilometer square.

A medium city tile is (2 x 16m) x 64 = 2048 meters or 2 kilometers square.

A large city tile is (4 x 16m) x 64 = 4086 meters or 4 kilometers square.

A foot equals 0.30479999798832 meters.

A meter equals 3.280839890131234 feet.

A mile equals 1.60934 kilometers.

A kilometer equals 0.621371 miles.

Consequently (round up to the nearest tenth):

A single grid square in a city is 52.5 feet square.

A small city tile is 3,484.8 feet or approximately .66 miles square.

A medium city tile is 6719.2 feet or 1.27 miles square.

A large city tile is 13,438.3 feet or 2.55 miles square.

    I always understood a kilometer to equal 1,000 meters, not 1024 meters.

    1,024 meters = 0.636 miles according to my calculations, but I could be wrong.
