How do you usually plan your city's commercial areas?Do you zone from the beginning 4x4 or 4x5(or more)areas or you just wait until your commercial demand grows and then rezone the areas?

Along main roads or little corner shops.

Usually I just zone where it needs to be, along heavily trafficked roads. Then, when the time comes for a more cogent downtown, I start slowly upgrading the density and sometimes the zone size, building-by-building. But I never start out with zones that won't develop for years into the city's future, because nothing would grow.
