It's up to you what size cities you want to place in your region. And it doesn't affect the city's importance. What does 'importance' in SC4 mean anyway??

    It does depend what you think importance is.  If it's a large population, well of course you'd need lots of people.  If importance is a large airport or a giant highway system for example, the circumstances are different, so really, it is up to you and what you think is important.

My region has only two large tiles, one for the central city, and one for the largest airport.

I guess to answer the question of the topic:  NO, city size has no affect on importance.  what you put in the city does however, and that is what the whole game is about!
